New Patient Registration
We only accept new patients living within the Practice boundary area. If you have any doubts please speak to a receptionist. If you wish to register please complete the forms listed below. When you have completed all of the details, print the form and sign and date where appropriate. When you visit the surgery our receptionist will check that all fields have been completed correctly and that the form has been signed at the bottom.
NB: PLEASE ENSURE YOUR NHS NUMBER IS ON THE REGISTRATION FORM.
If you do not have access to a printer, you can register by attending the surgery and completing the forms.
Patients on regular medication are asked to ensure they have a month's worth of medication in hand prior to registering. Patients will also need to provide proof of prescription upon registering. Shortly after, you will be contacted by our Practice Pharmacist for a brief review.
Once your registration has been signed and processed registered, you can then ask to speak to a doctor if you so require. Please see our appointments section on this website for further information.

NB: The Surgery maintains the right to change the practice boundary as is deemed necessary under its contract with the Health Board to maintain the high standard of care offered to existing patients.
Patient Removals
ALL patients who move outside of the stated Practice boundary, will be asked to find a new surgery. We understand that this can cause some distress to some patients but unfortunately we need to be very strict on this so as to ensure that we both maintain patient safety and comply with our General Medical Services (GMS) Contract by being able to provide a home visit to the property if needed.
